Forms
Under Local Rule VII a Praecipe and Docketing Statement are required to be filed with the Notice of Appeal. The forms can be obtained at the Clerk of Courts Office or downloaded below. It is essential that the Praecipe be signed by the Court Reporter to assure that the transcript has been ordered. Failure to file these forms may result in dismissal of the appeal:
